Once the sausage is cooked, reduce the heat to low and sprinkle the flour over the sausage. Stir well to combine and cook for about 2 minutes to eliminate the raw flour taste.
Gradually add the milk to the skillet, whisking continuously to prevent lumps. Continue stirring until the gravy thickens, about 3-5 minutes.
Add black pepper, salt, garlic powder, and onion powder (if using). Taste and adjust seasoning as needed.
